Please fill out the return section of the form completely and enclose it with the item(s) you are returning. Please indicate if you would like to receive a refund or make an exchange for another size, color or item. If you are exchanging merchandise, please fill out the exchange portion on the back of the form. Exchanges are subject to additional shipping charges.
